  • born in Edmonton, AB. Matric. Victoria High School, Edmonton. Interested in hockey and later played for RCAF teams in Canada. Worked in Innisfree, AB.as a mechanic. Enl. Edmonton Dec 1940. Trained at Brandon, High River, Patricia Bay. Overseas to England 5 Sep. 1941. Was Captain of his bomber crew. Mentioned in dispatches July 1942. While taking part in a combat exercise, 425 Sqn Wellington BJ695 underwent a dummy attack by a RCAF403 Sqn Spitfire AR440 at about 1000 feet, a practice conducted many times before. The Spitfire did not break away in time and struck the Wellington, sending itdown in a dive to hit a railway line nose first near the Black Bull pub between Pickering and Malton. All five crewmen died, either upon impact or from the fire that ensued. An army dispatch rider was first at the scene but could do nothing to save anyone. The Spitfire also crashed killing pilot F/Sgt JT Norman after breaking in two. Kuzyk was survived by parents, Mr. Mrs. Alexander Kuzyk, brother, Victor, in RCAF and sister, Nellie, all of Innisfree, AB.
